Use website forms for general information only

Qali Care’s forms are designed to collect contact details and a brief description of the service, referral, or employment interest. Do not send Social Security numbers, diagnoses, medical records, detailed medical histories, financial account information, or other highly sensitive information.

How submissions are delivered

Form information is validated by the website server and delivered to Qali Care only when a secure provider adapter has been configured. If delivery is not configured or fails, the website provides an error and does not claim that your information was received.

Resume files

Career applicants may attach a PDF, DOC, or DOCX file up to 2 MB. File delivery, retention, and deletion depend on the approved provider configuration and require owner confirmation before launch.
